  • Form P60 showing your salary and tax deducted in respect of your last employment in the year [together with form(s) P45 if you had more than one employment during the year]. If the forms are not available, a copy of your monthly or last week’s payslip before 5 April 2018 should be forwarded;
  • Details of any student loan repayments made during the year (including a note of which student loan repayment plan you are on) and if you believe your student loan will be fully repaid within the next two years;
  • Details of interest received from the bank and building society accounts, supported by interest certificates;
  • Details of dividend income, supported by dividend counterfoils and similar vouchers for fixed interest stocks;
  • The amount of state pension received in the year, together with the current rate of payment;
  • Details of other pensions received in the year;
  • Details of any other income arising during the year;
  • Details of pension contributions paid in the year;
  • Details of charitable covenants paid during the year;
  • Details of payments under Gift Aid during the year;
  • Details of any underpaid tax for earlier years included in your tax code for this year (and if known) for the following year;
  • A copy of the form(s) P11D showing benefits in kind provided to you in connection with your employment(s) during the year;
  • SELF EMPLOYMENT: Sole trader – Please forward the books and records of the business for the accounting period ended during the tax year, if not already provided. If this firm does not produce accounts for the business please provide a copy of the accounts, and tax computations if available, for the accounting period ended during the tax year;
  • SELF EMPLOYMENT: Partnership – Please forward the books and records of the business for the accounting period ended during the tax year, if not already provided. If this firm does not produce accounts for the business please provide a copy of the Partnership Statement;
  • Details of rents received and expenses in respect of any let properties;
  • Forms R185/R185(Trust) showing your income and tax deducted in respect of any settlement of which you are a beneficiary;
  • Capital gains – Please provide details of any assets (including shares) disposed of during the year, including dates of purchase and sale, proceeds, acquisition costs, enhancement costs and any costs of sale. Please supply the contract notes or transaction/valuation documents if held. If you have disposed of land or property, or unquoted shares, please inform us as soon as possible as valuations may be required to be made and agreed with HM Revenue & Customs;
  • Chargeable event certificates on certain bonds and life assurance policies;
  • Certificate of interest paid in respect of any other loan on which you obtain, or a new loan which you consider that you may be entitled to, tax relief, with details of the purpose of any new loan;
  • Details of any Child Benefit you and your partner received during the year (including a note of the number of children for which this was paid and a note of your partner’s income if it was higher than £50,000 for the year);
  • If you have applied for a Marriage Allowance transfer, either to or from your spouse or civil partner;
